About this emergency contact form

An emergency contact form lists the people to call if something happens to you, in order, with their relationship and phone numbers, plus the few medical facts responders need: blood group, conditions, allergies, medication and your doctor. Gyms, clubs, landlords, schools and employers ask for one, and it’s worth keeping in your wallet or travel bag too.

Employers and schools have dedicated versions; this one is for personal use and any organisation.

What’s on the emergency contact form

The template has 4 sections and 11 questions. Every one can be renamed, moved or deleted.

  • Your details: Full name; Date of birth; Phone; Address
  • Emergency contacts (call in this order): Contacts table (Name, Relationship, Mobile, Other phone)
  • Medical: Blood group; Doctor’s name and phone; Medical conditions; Allergies; Medications
  • Signature: Signature with date

“In Case of Emergency”: responders look for ICE contacts. Most phones can show emergency contacts and medical information on the lock screen.
