About the custom form builder
The form templates on Atifo are quick starting points; this builder is for when you want complete control. Start with a blank page and add exactly the elements you need: short and long answers, writing lines, numbers, dates, character boxes for ID numbers, single choices, tick boxes, dropdowns, yes/no questions, rating scales, agreements, tables, signatures, headings, paragraphs, dividers, spacing, images and page breaks.
Every element has its own settings. Make it a quarter, third, half, two-thirds, three-quarters or the full width of the page, so several fields sit side by side. Put its label above, beside or inside the box, or hide it. Choose a box, an underline or blank space for the answer, add help text, mark it required, and list choices side by side or one per line.
Fillable or print only
A fillable PDF has real form fields: people type into boxes, tick checkboxes, pick from dropdowns and save or print the completed form in Chrome, Edge, Acrobat Reader and most phone PDF apps. A print-only PDF has the same design without fields, ready to photocopy and fill in by hand (dropdowns become “choose one” options on paper).
Word gives you an editable .docx; HTML gives you code for your own website with labels tied to inputs and a layout that adapts to phones.
Design the whole page
- Text size: small to extra large, for forms that need to be easy to read.
- Labels: above, beside or inside the answer, for the whole form or each field.
- Answers: boxes, underlines or blank space.
- Sections: coloured bars, underlined titles, plain titles or bordered boxes, each with an optional description; titles can be hidden.
- Page: portrait or landscape, A4 or US Letter, narrow to wide margins, a page border, header and footer text, page numbers and your date format.
- Branding: logo, organisation name, contact line and any colour.
Save your design and keep editing
Your form is saved in this browser automatically, so you can close the page and come back. Use “Save design” to download a small .json file you can open again later, on any device, to keep editing or make a new version. Nothing you type or upload ever leaves your device.

Frequently asked questions
Can I place fields side by side?
Yes. Set each element’s width to ¼, ⅓, ½, ⅔, ¾ or full, and elements fill each row from left to right, like a 12-column grid.
Can I remove labels or legends?
Yes. Hide any label, put it beside or inside the box, or hide a section title while keeping the section’s grouping and style.
Are my forms or answers uploaded?
No. The builder runs entirely in your browser. Your design is saved only on this device, and filled-in PDFs stay with whoever fills them in.
Can people fill in the PDF on a phone?
Yes. Fillable PDFs work in most phone PDF apps, Chrome and Edge. Typed answers can be saved and shared, or the form printed.
Can I start from an existing form?
