About this fitness assessment form
Personal trainers, gyms, sports coaches and PE teachers assess fitness at the start of a program and every few weeks after. This form records goals, relevant health history, body measurements, resting heart rate and blood pressure, and results for push-ups, sit-ups, sit-and-reach, plank and a cardio test, with columns for a re-test.

What’s on the fitness assessment form
The template has 5 sections and 13 questions. Every one can be renamed, moved or deleted.
- Client: Name; Age; Sex (choose one: Female, Male, Other); Phone; Trainer; Date
- Goals and health: Goals (tick all that apply: Lose weight, Build strength, Improve fitness, Sport performance, Rehab, General health); Injuries or health conditions; Current activity level (choose one: Sedentary, Light, Moderate, Active, Very active)
- Measurements: Measurements table (Measure, Baseline, Re-test 1, Re-test 2)

Tips
- Test at the same time of day and in the same conditions each time.
- Record exactly which test you used (e.g. 3-minute step test).
- Celebrate progress against the client’s own baseline, not others.

How often should fitness be re-assessed?
Every 6 to 12 weeks is typical, often enough to see change and adjust the plan.
Do I need medical clearance first?
If screening shows heart, blood pressure or other concerns, ask the client to see a doctor before testing.
Can I change the questions on the fitness assessment form?
Yes. Rename any question, change its answer type (short answer, tick boxes, rating, table and more), move it, or delete it. You can also add whole new sections.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
