About this donation form

A donation form records who gave, how much, how often, for what cause and how they paid, and asks whether they want a receipt for tax purposes and how they’d like to be acknowledged. NGOs, schools, places of worship, community groups and fundraisers use it at events, in offices and online.

In India, donors to eligible organisations can claim a deduction under Section 80G with a proper receipt; the form asks for PAN, which is required for those receipts. Issue the receipt itself with the receipt generator.

What’s on the donation form

The template has 4 sections and 16 questions. Every one can be renamed, moved or deleted.

  • Donor: Full name; Phone; Email; Address; PAN (for tax receipts)
  • Your gift: Amount (choose one: 500, 1,000, 2,500, 5,000, 10,000, Other); Other amount; Frequency (choose one: One-time, Monthly, Yearly); Purpose (choose one: Where it’s needed most, Education, Health, Food, Building fund, Other); In memory or honour of (optional)
  • Payment: Method (choose one: UPI, Bank transfer, Cheque, Cash, Card); Transaction or cheque number; Date

  • Offer suggested amounts; they make giving easier.
  • Ask permission before publishing donors’ names.
  • Thank donors within a few days with a thank-you card.

Four ways to save it

Frequently asked questions

Why ask for PAN on a donation form?

In India, organisations issuing 80G receipts must report donations with the donor’s PAN. Donors who don’t need a tax receipt can leave it blank.

Can donors pay online?

Add your UPI ID or a payment link in the instructions; the form records the method and transaction reference.

Can people fill in this donation form on a phone?

Yes. Choose “Fillable PDF” and send the file. It opens in the Files or Drive app on Android, Files on iPhone, and any browser on a computer, where people can type their answers and save the result.

Can I add my logo to the donation form?

Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.

Is anything I type sent to a server?

No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
