About this health goal tracker
Health goals such as lowering blood pressure, losing weight or sleeping better are reached through daily habits. This tracker puts each goal next to the habit that supports it, then gives a box for every day of the month to tick.
At the end of the month, review what worked and set the next step.
What’s on the page
- My goals: a table with columns for Goal, Target and date, Daily habit.
- Daily habits: a tracker grid with a row for your daily habits for my goals and a box for each day of the month.
- Labelled boxes for What worked, Next month.
Tips
- Pair every outcome goal with a daily action.
- Check in with your doctor on medical targets.
- Celebrate streaks, not just results.
Made to fit, then printed
Everything on this health goal tracker can be changed before you print: the title, the daily habits for my goals, the dates, the colour, the paper size and orientation. The page is drawn on your device and nothing is uploaded. Find more printables.
Frequently asked questions
How many health goals should I set?
One to three at a time works best. The tracker has room for five if you need it.
Can I fill in the health goal tracker on my phone or tablet?
Yes. Switch on “Fillable PDF” before downloading and the PDF gets typing boxes and tick boxes you can use in most PDF apps. Or print it and use a pen.
Can I print several months at once?
Yes. Pick the start date and how many months you need; each month gets its own page with the right dates, all in one PDF.
Can I change the daily habits for my goals?
Yes. Type your own, one per line, and the page updates as you type. Leave a line empty to remove it.
Can I remove the atifo.app line at the bottom?
Yes. Switch off “Show atifo.app credit” in the Look section and the PDF has no branding at all.
