About this pet information form
Leaving a pet with a sitter, a kennel or a friend? A pet information sheet tells them what they need: the pet’s name, breed and microchip number, your vet and an emergency vet, vaccinations, food and portions, medication, walks and litter, and behaviour quirks like “doesn’t like other dogs”.
It’s also useful if a pet goes missing; keep a copy with a recent photo.
What’s on the pet information form
The template has 4 sections and 14 questions. Every one can be renamed, moved or deleted.
- Owner: Owner name; Mobile; Back-up contact and phone; Home address
- Pets: Pets table (Name, Species and breed, Age, Microchip no.)
- Vet: Vet name and phone; Emergency vet and phone; Vaccinations up to date? (yes or no); Agreement: “The sitter may arrange vet treatment up to an agreed amount.”
- Care: Food and portions; Feeding times; Medication; Walks, play or litter routine; Behaviour notes (fears, habits, other animals)

- Write portions in cups or grams and exact feeding times.
- Leave written permission for the sitter to arrange vet treatment.
- Include a recent photo and the microchip number.
Four ways to save it
A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. Can I list more than one pet?
Yes. The pet table has room for several animals.
Can I add my logo to the pet information form?
Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.
Can I get the pet information form as a Word document?
Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
