About this child information form
A child information form tells a daycare, preschool, camp, nanny or relative what they need to care for a child well: parents’ contacts, who may pick the child up, health and allergies, sleep and meal routines, toilet training, comfort items and what helps when they’re upset.
Keep it updated as your child grows; routines and allergies change.
What’s on the child information form
The template has 6 sections and 16 questions. Every one can be renamed, moved or deleted.
- Child: Full name; Preferred name; Date of birth; Language(s) spoken at home
- Parents: Parents table (Name, Relationship, Mobile, Email)
- Pickup: People allowed to collect my child

What should I tell a daycare about my child?
Contacts, pickup authorisation, health and allergies, medication, routines (naps, meals, toilet), and anything that comforts or worries your child.
How is this different from the babysitter form?
The child form is about the child and suits daycares and camps. The babysitter form is about your home: house rules, bedtime and where things are.
Can I put the child information form on my website?
Yes. Choose “HTML” to copy ready-made form code with labels and a simple style. Set the form’s action to the address that should receive submissions, such as your own form handler.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
Can I get the child information form as a Word document?
