About this vision screening form

Schools, eye camps and community health programs run vision screening to catch children and adults who need a full eye examination. This form records visual acuity for each eye with and without glasses, symptoms such as squinting or headaches, colour vision, the outcome and whether the family was notified about a referral.

Screening isn’t a diagnosis; anyone who doesn’t pass should see an optometrist or ophthalmologist.

What’s on the vision screening form

The template has 3 sections and 13 questions. Every one can be renamed, moved or deleted.

  • Test each eye separately at the distance your chart requires.
  • Record the chart used (e.g. Snellen, LogMAR, tumbling E).

Frequently asked questions

What does 6/6 or 20/20 mean?

Normal distance vision: at 6 metres (20 feet) you see what a typical eye sees at that distance. 6/12 means you see at 6 metres what others see at 12.

Who should be referred?

Follow your program’s rules, commonly anyone worse than 6/9 in either eye, a difference between eyes, or symptoms.

Can I change the questions on the vision screening form?

Yes. Rename any question, change its answer type (short answer, tick boxes, rating, table and more), move it, or delete it. You can also add whole new sections.

Is anything I type sent to a server?

No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
