About this student information form

Teachers send a student information form home in the first week of the year to learn about each child: how to reach the family, who may pick them up, health needs, how they learn best and what they enjoy. It’s shorter than the school’s admission form and written for the class teacher.

Families can type into the fillable PDF on a phone and send it back, which saves deciphering handwriting.

What’s on the student information form

The template has 6 sections and 14 questions. Every one can be renamed, moved or deleted.

• Student: Full name; Preferred name; Date of birth; Class / grade
• Family contacts: Contacts table (Name, Relationship, Phone, Email); Best way to contact you (choose one: Phone, WhatsApp, Email, Note in diary)
• Pickup: People allowed to pick up my child; How does your child get home? (choose one: Walks, School bus, Car, Public transport, After-school care)
• Health and learning: Allergies or medical needs; How does your child learn best?; Anything that worries your child about school?
• About your child: Interests and hobbies; What else should I know about your child?
• Signature: Parent or guardian signature with date

Tips

• Ask one open question such as “What should I know about your child?”; it often gives the most useful answer.
• Keep completed forms in a confidential folder.
• Build your class contact list from the returned forms.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper.
