About this parent consent form
Schools, sports clubs, camps and youth groups need a parent’s written consent before children take part in many activities, appear in photos or receive first aid. A parent consent form explains the activity, records the parent’s agreement and collects the medical and contact details staff need on the day.
Photo consent is asked separately, because many families are happy for their child to join an activity but not to appear online.
What’s on the parent consent form
The template has 5 sections and 17 questions. Every one can be renamed, moved or deleted.
- Child: Student name; Class / grade; Date of birth
- Activity: Activity; Date(s); Location; Supervising staff; Description and any risks
- Consent: Agreement: “I give consent for my child to take part in the activity described.”; Agreement: “In an emergency, I consent to first aid and medical treatment being given if I cannot be reached.”; Photos or videos of my child may be used in school publications and on the school website. (yes or no); Photos may be shared on social media. (yes or no)
- Medical and contact: Allergies, conditions or medication; Parent or guardian name; Phone; Alternative contact and phone

- Send the form with enough notice for families to ask questions.
- Carry completed forms (or copies) on the activity itself.
