About this declaration form
A declaration form is a signed statement that certain facts are true: a self-declaration of address or income for an application, a declaration of no criminal record for volunteering, a health declaration before an event, or a declaration of conflict of interest. The signer takes responsibility for the truth of what they state.
Replace the sample statement with the facts you need declared. The form includes space for supporting documents and an optional witness, which some organisations require.

- Keep the statement factual and specific; vague declarations are hard to rely on.
- If an authority asks for a sworn affidavit, a self-declaration is usually not enough; it needs a notary or oath commissioner.
- Check whether the receiving office has its own required format.

Is a self-declaration the same as an affidavit?
No. A self-declaration is signed by you alone. An affidavit is sworn before an authorised official such as a notary, usually on stamp paper in India. Some applications accept self-declarations instead of affidavits.
What happens if a declaration is false?
Knowingly signing a false declaration can lead to an application being cancelled and, in some cases, legal penalties. Only sign what you know to be true.
Can people fill in this declaration form on a phone?
