About this waiver form
A waiver (or release of liability) asks participants to confirm they understand the risks of an activity and accept them. Gyms, adventure sports, fitness classes, equipment hire, sports leagues and volunteer work days commonly use one.
This template sets out the activity and its risks, an assumption-of-risk and release statement, permission for emergency medical treatment, and signatures, including a parent or guardian for minors. Waiver law varies a lot between countries and states, and many places don’t allow releasing liability for negligence, so have the wording reviewed locally.
What’s on the waiver form
The template has 4 sections and 16 questions. Every one can be renamed, moved or deleted.

Tips
- Describe the real risks in plain words; a waiver nobody understands protects nobody.
- Ask about health conditions separately with the health screening form if the activity is strenuous.
- Keep signed waivers for as long as a claim could be made where you operate.

Frequently asked questions
Is a waiver enforceable?
It depends on the law where you operate and how the waiver is written. Many jurisdictions won’t enforce a waiver for gross negligence, and some won’t enforce them for consumers at all. Get local legal advice.
Can a parent sign a waiver for a child?
Parents commonly sign for minors, and the template includes that section, but some places limit how far a parent can waive a child’s rights.
Can I add my logo to the waiver form?
Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.
Can I get the waiver form as a Word document?
Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
