About this consent form
A consent form records that someone understood what they were agreeing to and said yes freely. It’s used for activities, treatments, research, photography and sharing personal information. Good consent forms explain the purpose, the risks and the right to withdraw in plain language.
This template has space to describe the activity, confirm understanding, choose optional extras (such as photos) separately, and sign. A guardian section covers participants under 18. Replace the sample wording with a clear description of your own activity.
What’s on the consent form
The template has 4 sections and 15 questions. Every one can be renamed, moved or deleted.

- What you are agreeing to: Activity, treatment or purpose; Date(s); Location; Agreement: “I have been told what this involves, including any risks, and I have had the chance to ask questions.”; Agreement: “I understand that taking part is voluntary and that I can withdraw my consent at any time.”

Tips
- Write the description for the person signing, not for lawyers: short sentences, no jargon.
- Ask for optional things (photos, contact afterwards) as separate tick boxes, never bundled into the main consent.
- Give the signer a copy; with a fillable PDF that’s as easy as sending the completed file back.
Four ways to save it

Frequently asked questions
Is this consent form legally binding?
It records consent, but whether it’s enough depends on your country and situation. Medical, research and activities with children often have specific rules; have the wording checked before relying on it.
What’s the difference between consent and a waiver?
Consent says “I agree to take part”. A waiver goes further and says “I accept the risks and won’t hold you responsible”. Use the waiver form if you need that.
Can people fill in this consent form on a phone?
Yes. Choose “Fillable PDF” and send the file. It opens in the Files or Drive app on Android, Files on iPhone, and any browser on a computer, where people can type their answers and save the result.
Can I add my logo to the consent form?
Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
