About this authorization form

An authorization form gives another person written authority to act for you: to collect documents, a parcel or a child from school; to speak to a bank, school or utility on your behalf; or to sign for a delivery. It names both people, describes exactly what is allowed and for how long.

Both signatures and ID details help the receiving office check that the authorised person is who they say they are. For card payments, use the payment authorization form instead.

What’s on the authorization form

The template has 4 sections and 15 questions. Every one can be renamed, moved or deleted.

Tips

  • Limit the authority to what’s needed: one collection on one date is safer than open-ended authority.
  • Ask the authorised person to bring their original ID when they act on the form.
  • Many banks and government offices have their own authorization forms; check before you go.

Frequently asked questions

Is an authorization form the same as a power of attorney?

No. An authorization covers a specific task. A power of attorney is a formal legal document granting broad powers and usually needs registration or notarisation.

Should I attach a copy of my ID?

Often yes. Write “Copy of ID attached” on the form and sign across the photocopy to show it’s for this purpose only.

Can I change the questions on the authorization form?

Yes. Rename any question, change its answer type (short answer, tick boxes, rating, table and more), move it, or delete it. You can also add whole new sections.

Can I get the authorization form as a Word document?

Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.

Can I put the authorization form on my website?
