About this permission form
A permission form puts “yes, you may” in writing: permission for a child to join an outing, for a neighbour to use a driveway, for a photographer to shoot on your property, or for a group to use a hall. It names who is giving permission, to whom, for what, and under which conditions.
For school trips and events with children, the field trip and event permission forms include medical and contact questions that schools need. This general version suits everything else.
What’s on the permission form
The template has 4 sections and 14 questions. Every one can be renamed, moved or deleted.

Tips
- Be specific about dates and limits: “Saturday 14 June, 9 am to 5 pm” is clearer than “the weekend”.
- List any conditions (insurance, supervision, clean-up) so both sides know what was agreed.
- Keep a signed copy each.

Is a permission slip the same as a permission form?
Yes. Schools usually say “permission slip” for short forms parents sign. The field trip permission form is designed for that use.
Does a permission form need a witness?
Usually not, but a witness signature adds weight when the permission concerns property or valuable items. The witness section is optional; delete it if you don’t need it.
Can I get the permission form as a Word document?
Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.
Can people fill in this permission form on a phone?
