About this information form

An information form puts a person’s key details on one tidy page: name, contact information, address, ID and who to call in an emergency. Landlords, tutors, coaches, volunteer coordinators and small offices use it to keep records consistent instead of collecting details on scraps of paper.

Ask only for what you genuinely need. If you don’t need an ID number or date of birth, delete those fields; collecting less personal data is safer for you and the people you serve.

What’s on the information form

The template has 4 sections and 17 questions. Every one can be renamed, moved or deleted.

• Personal details: Full name; Preferred name; Date of birth; Nationality; ID type and number; Occupation
• Contact details: Mobile number; Alternative number; Email address; Preferred contact method (choose one: Phone, Email, WhatsApp / SMS, Post); Address
• Emergency contact: Name; Relationship; Phone number; Email address
• Consent: Agreement: “I agree that these details may be stored and used for the purpose explained to me.”; Signature with date

Tips

• Store completed forms securely and dispose of them when you no longer need them.
• Tell people why you need their details in the instructions box.
• For staff or students, use the employee or student information forms, which add role and class details.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper.
