About this intervention tracker
When a student gets extra support, it’s important to record what was done and whether it worked. This tracker sets out the target skill, the intervention, its frequency and who delivers it, then logs each session with a score so progress is visible and decisions are based on evidence.
It supports response-to-intervention (RTI), remedial teaching and special educational needs records.
What’s on the page
- A header with student, class, start date, target skill, intervention, frequency.
- Session log: a table with columns for Date, Activity, Score, Notes, Staff.
- Labelled boxes for Review and decision, Next steps.
Tips
- Measure the same way each session.
- Review after 6–8 weeks and decide: continue, change or stop.
- Share progress with the family.
Made to fit, then printed
Everything on this intervention tracker can be changed before you print: the title, the colour, the paper size and orientation. The page is drawn on your device and nothing is uploaded. Find more printables.
Frequently asked questions
What is RTI?
Response to Intervention: a tiered approach where students get increasingly targeted support, with progress monitored to guide decisions.
What paper size should I print on?
Choose A4 or US Letter before downloading, and print at 100% or “Actual size”. Portrait and landscape layouts are both available.
Will it print well in black and white?
Yes. The layout uses light fills and clear lines that print cleanly in grey. Choose the “Grey” colour to save colour ink entirely.
Can I remove the atifo.app line at the bottom?
Yes. Switch off “Show atifo.app credit” in the Look section and the PDF has no branding at all.
Can I fill in the intervention tracker on my phone or tablet?
Yes. Switch on “Fillable PDF” before downloading and the PDF gets typing boxes and tick boxes you can use in most PDF apps. Or print it and use a pen.
