About this order form

An order form lets customers tell you exactly what they want: which items, how many, delivery or pick-up, and how they’ll pay. Home bakers, small shops, uniform suppliers, school fundraisers and craft sellers rely on it to avoid mix-ups.

The template has customer details, an itemised table with quantity, unit price and amount, delivery details and payment options. To bill the customer afterwards, make an invoice with the invoice generator, which totals items and taxes for you.

What’s on the order form

The template has 3 sections and 12 questions. Every one can be renamed, moved or deleted.

  • Customer: Name; Phone; Email; Order date; Delivery address
  • Order: Items table (Item, Size / variant, Qty, Unit price, Amount); Total amount
  • Delivery and payment: Delivery or pick-up (choose one: Delivery, Pick-up); Required by; Payment method (choose one: Cash, UPI, Bank transfer, Card, Cheque); Special instructions; Customer signature with date

Tips

  • Print your product list and prices in the table before handing out paper forms.
  • Add a “Deposit paid” line in the office box if you take part-payment.
  • Confirm each order back to the customer with an invoice or receipt.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. Frequently asked questions

Does the form calculate totals?

No; it’s for customers to fill in. For automatic totals and taxes, use the invoice or quotation generator once the order is confirmed.

Can I list my products in advance?

Yes. In a fillable PDF, type your products into the first column before sending it. On paper, write them in before printing copies.

Can I add my logo to the order form?

Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.

Can I get the order form as a Word document?

Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.

Is anything I type sent to a server?

No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
