About this testimonial form
Testimonials are some of the most convincing words on a website or flyer, but only when they’re genuine and you have permission to use them. A testimonial form asks happy customers the right questions to get a specific, believable quote and records exactly how you may use it.
The prompts follow a simple story: what the customer needed, why they chose you, what changed. The permission section lets them choose whether their full name, first name only, business name or photo appears with the quote.
What’s on the testimonial form
The template has 3 sections and 13 questions. Every one can be renamed, moved or deleted.

Tips
- Ask right after a success, when customers are happiest.
- Edit for length only, never meaning, and send the shortened version back for approval.
- In many countries, fake or paid-for reviews without disclosure are illegal; only publish real ones.
Four ways to save it

Do I need written permission to use a testimonial?
It’s best practice everywhere and required in many places, especially with a name or photo. The form records exactly what the customer agreed to.
How do I get more specific testimonials?
Ask about the situation before, the result after, and one detail they liked. Specific results (“saved me three hours a week”) persuade far more than “Great service!”.
Can I change the questions on the testimonial form?
Yes. Rename any question, change its answer type (short answer, tick boxes, rating, table and more), move it, or delete it. You can also add whole new sections.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
Can people fill in this testimonial form on a phone?
