About this sign-up form
A sign-up sheet collects names from lots of people on one page: at a market stall, a school fair, a meeting, a volunteer drive or the start of a club season. People queue less when there’s no individual form to fill in.
This version has a table across the page with name, email, phone and a tick box for receiving updates, so you only contact people who said yes. Rename the columns for shift slots, items to bring or T-shirt sizes.

Tips
- Write the purpose clearly at the top so people know what they’re signing up for.
- Print a few copies and attach them to a clipboard with a pen on a string.
- Type the list into a spreadsheet afterwards and destroy the paper copy.

How many people fit on one sign-up sheet?
About 20 to 25 rows fit on an A4 or Letter page. The table fills the page automatically, and you can print as many pages as you need.
Can I use this as a volunteer sign-up sheet?
Yes. Rename a column to “Shift” or “Task”, or use the volunteer registration form for a full individual form.
Can I put the sign-up form on my website?
Yes. Choose “HTML” to copy ready-made form code with labels and a simple style. Set the form’s action to the address that should receive submissions, such as your own form handler.
Can I change the questions on the sign-up form?
Yes. Rename any question, change its answer type (short answer, tick boxes, rating, table and more), move it, or delete it. You can also add whole new sections.
Can I get the sign-up form as a Word document?
Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.
