About this data collection form

A data collection form makes sure every observation is recorded in the same way, whether you’re counting traffic, logging water quality, auditing stock, recording behaviour in a classroom study or gathering field data for a school project. Consistent records are what make data trustworthy.

The template has a header for the project, site and collector, a recording table with date, time, measurement and notes columns you can rename, and space for conditions and anything unusual.

How many rows does the table have?

It fills the rest of the page by default. Set an exact number of rows in the question settings, or add a second table for a different measurement.

Is this suitable for student science projects?

Yes. Rename columns to the variables in the experiment and use the notes box for observations.

Can I get the data collection form as a Word document?

Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.

Can people fill in this data collection form on a phone?
