About this nomination form
A nomination form puts someone forward for an award, an election, a committee role or a recognition scheme such as employee of the month. Good nominations say not just that someone is great, but what they did and what difference it made.
This template records the nominee, the category, the nominator and a seconder, and asks for specific examples and impact. A tick box confirms the nominee has agreed to stand, which elections usually require.
What’s on the nomination form
The template has 5 sections and 15 questions. Every one can be renamed, moved or deleted.

Tips
- List your award categories in the category question so nominators tick one.
- Ask for examples from a defined period, such as “in the last 12 months”.
- Once winners are chosen, make their certificates with the award certificate maker.
Four ways to save it
A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. Frequently asked questions
Do nominees need to agree to a nomination?
For elections and committee roles, usually yes. For surprise awards, delete the consent tick box.
Can someone nominate themselves?
That’s up to your rules. If self-nomination is allowed, say so in the instructions and keep the seconder section.
Can I add my logo to the nomination form?
Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
Can people fill in this nomination form on a phone?
Yes. Choose “Fillable PDF” and send the file. It opens in the Files or Drive app on Android, Files on iPhone, and any browser on a computer, where people can type their answers and save the result.
