About this classroom survey form

A classroom survey is a teacher’s own quick check-in with students: Do they feel they belong? Is the pace right? Do they prefer working alone or in groups? What helps them learn? It’s short, uses friendly language and works from about age eight up.

Run it a few weeks into the year and again mid-year to see whether changes made a difference.

What’s on the classroom survey form

The template has 2 sections and 7 questions. Every one can be renamed, moved or deleted.

  • How I feel: I feel welcome in this class. (rated Not really to Yes!); I can ask for help when I need it. (rated Not really to Yes!); The work is the right level for me. (rated Too easy to Too hard); The pace of lessons is (choose one: Too slow, Just right, Too fast)
  • How I learn: I learn best when I (tick all that apply: Work alone, Work with a partner, Work in a group, Watch a demonstration, Do hands-on activities, Read about it); My favourite thing we did this term; Something that would help me learn better

Tips

  • Tell students there are no wrong answers and you won’t be upset.
  • Let younger students colour the faces instead of ticking.
  • Talk about one thing you’ll change because of their answers.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. The Word file can be edited further, and the HTML code adds the same questions to a website. Browse more form templates, or design a completely different layout in the custom form builder.
