About this student recommendation form

Selective programs, scholarships and colleges ask teachers to recommend students. A recommendation form makes it easier for busy teachers: rate the student against peers on academic ability, motivation, character and leadership, then add a paragraph with examples.

The ratings compare the student with others the teacher has taught, which panels find especially useful.

What’s on the student recommendation form

The template has 4 sections and 15 questions. Every one can be renamed, moved or deleted.

Tips

  • Give teachers at least two weeks.
  • Students can attach a short note of their activities to help the teacher write.
  • Ask teachers to return the form directly to the program.

Four ways to save it

A teacher who knows them well, ideally in a subject related to the application, rather than the most senior person.

Is the recommendation confidential?

Usually yes. Add a line in the instructions asking the referee to send it directly.
