About this student self-assessment form

Self-assessment helps students take ownership of their learning. This form asks them to rate their effort, understanding, organisation and behaviour, then reflect on what they’re proud of, what was hard and what they’ll do next.

Teachers often use it before report cards or conferences, then compare it with their own evaluation; differences make for useful conversations.

What’s on the student self-assessment form

The template has 3 sections and 13 questions. Every one can be renamed, moved or deleted.

What age is self-assessment suitable for?

From about age seven with simple scales. Older students can reflect in more depth.

Should self-assessment be graded?

No. It’s for reflection; grading it encourages students to rate themselves higher.

Can I get the student self-assessment form as a Word document?

Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.

Can people fill in this student self-assessment form on a phone?
