About this course feedback form
A course feedback form asks students to rate a course at the end: whether the objectives were clear, how good the content and materials were, whether the workload and assessments were fair, and what they’d change. Colleges, coaching institutes, online course creators and workshop leaders use it to improve the next run.
Ask the same core questions every time so you can compare courses and track improvement.

Tips
- Collect feedback before final grades are released to avoid bias.
- Ask for one specific suggestion rather than general comments.
- Thank students and show what changed from last year’s feedback.
Four ways to save it
A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. The Word file can be edited further, and the HTML code adds the same questions to a website. Browse more form templates, or design a completely different layout in the custom form builder.
Frequently asked questions
How is this different from a teacher evaluation?
Course feedback focuses on the course: content, materials, workload, assessments. Teacher evaluation focuses on the teaching itself.
Can I use it for an online course?
Yes. Use the HTML version on your course site or send the fillable PDF.
Can I get the course feedback form as a Word document?
Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
Can I put the course feedback form on my website?
Yes. Choose “HTML” to copy ready-made form code with labels and a simple style. Set the form’s action to the address that should receive submissions, such as your own form handler.
