About this client onboarding form
Once a client has signed, onboarding collects what you need to start: who to bill and how, who approves the work, brand files and logins, how they like to communicate and when to hold the kickoff meeting. Collecting it in one form stops weeks of scattered emails.
Never ask clients to write passwords on a form; ask them to share access through a password manager or invite your account instead.
What’s on the client onboarding form
The template has 5 sections and 17 questions. Every one can be renamed, moved or deleted.
- Company: Company legal name; Billing address; GSTIN / tax ID; Purchase order number (if needed)
- Contacts: Contacts table (Role, Name, Email, Phone); Main point of contact; Person who approves work
- Billing: Invoices should go to; Payment method (choose one: Bank transfer, UPI, Card, Cheque); Payment terms agreed
- Brand and access: Brand assets you will share (tick all that apply: Logo files, Brand guidelines, Fonts, Photos, Previous work); Accounts we need access to (no passwords here); Competitors or examples you like
- Working together: Preferred communication (tick all that apply: Email, Phone, WhatsApp, Slack or Teams, Video calls); Meeting frequency (choose one: Weekly, Fortnightly, Monthly, As needed); Kickoff meeting date; Anything else we should know?
Tips
- Send the form with your welcome email straight after the contract is signed.
- Confirm one main point of contact and one approver.
- Agree response times and meeting frequency in writing.
Four ways to save it
A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. The Word file can be edited further, and the HTML code adds the same questions to a website. Browse more form templates, or design a completely different layout in the custom form builder.
