A service request form is how customers ask for a repair, installation, inspection or other service. It captures the product and model, the problem in the customer’s own words, warranty details and when someone can visit, so the right technician arrives with the right parts.

What’s on the service request form

The template has 3 sections and 14 questions. Every one can be renamed, moved or deleted.

• Customer: Name; Phone; Email; Service address
• Request: Service needed (choose one: Repair, Installation, Maintenance, Inspection, Other); Product or system; Brand and model; Serial number; Under warranty? (choose one: Yes, No, Not sure); Purchase date; Describe the problem
• Visit: Preferred date; Preferred time (choose one: Morning, Afternoon, Evening); Access or parking notes

Tips

• Ask for the model and serial number; it often decides which parts to bring.
• Offer time windows rather than exact times.
• Send a confirmation with a reference number.
