About this client intake form

A client intake form helps agencies, freelancers, consultants, designers, accountants and lawyers decide whether a new client is a good fit and what the work will involve. It collects the business background, what they want to achieve, scope, budget, deadlines and who makes decisions, before you write a proposal.

Send it before the first call; the conversation is then about solutions instead of basic facts. For personal services such as therapy or coaching, the general intake form fits better.

What’s on the client intake form

The template has 4 sections and 19 questions. Every one can be renamed, moved or deleted.

Four ways to save it

What’s the difference between client intake and client onboarding?

Intake comes before you agree to work together and helps you scope and price the job. Onboarding comes after the contract and collects logins, assets and contacts to start the work.

Should I ask about budget on an intake form?

Yes. A budget range tells you whether the project is feasible and what to propose, and it saves time for both sides.

Is anything I type sent to a server?

No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.

Can I put the client intake form on my website?
