About this contact form

A contact form gives people one clear way to reach you and gives you the details you need to reply: who they are, how to contact them and what they want. Websites use it as a “Contact us” page, and shops, clinics and event stalls keep a paper version at the counter for enquiries that need a call back.

This template asks for the essentials and lets the person choose how they’d like you to reply. Add a “How did you hear about us?” question if you track marketing, or remove the phone number if you only reply by email.

What’s on the contact form

The template has 2 sections and 10 questions. Every one can be renamed, moved or deleted.

  • Your details: Full name; Email address; Phone number; Company or organisation
  • Your message: Subject; Reason for contacting us (choose one: General question, Quote or pricing, Support, Feedback, Other); Message; Preferred way to reply (choose one: Email, Phone call, WhatsApp / SMS); Best time to contact you (choose one: Morning, Afternoon, Evening); Agreement: “I agree to be contacted about my enquiry and understand my details won’t be shared with anyone else.”

Tips

  • Keep it short: every extra question lowers the number of people who finish the form.
  • For a website, download the HTML version and point the form’s action at your email or form handling service.
  • Ask for consent before adding enquirers to a mailing list; the tick box at the end does exactly that.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. Will the HTML contact form send me emails?

Not by itself. HTML forms need somewhere to send the answers. Set the form’s action to your website’s form handler or a form service, and it will deliver submissions to you.

What should a contact form include?

At minimum a name, a way to reply (email or phone) and a message box. A subject or reason helps you route enquiries, and a preferred-contact option saves a round trip.

Can people fill in this contact form on a phone?

Yes. Choose “Fillable PDF” and send the file. It opens in the Files or Drive app on Android, Files on iPhone, and any browser on a computer, where people can type their answers and save the result.

Can I put the contact form on my website?

Yes. Choose “HTML” to copy ready-made form code with labels and a simple style. Set the form’s action to the address that should receive submissions, such as your own form handler.

Is anything I type sent to a server?

No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
