About this feedback form

A feedback form is a short, friendly way to ask “How did we do?”. It works for a service, a class, a product, a visit or a meeting, and it’s quick enough that people fill it in on the spot.

This version opens with an overall rating, asks about a few specific areas, then gives space for what went well and what could be better. It ends by asking whether you may quote the comments and whether the person wants a reply, so positive feedback can become a testimonial and problems can be followed up.

What’s on the feedback form

The template has 3 sections and 11 questions. Every one can be renamed, moved or deleted.

  • Your rating: Overall experience (rated Poor to Excellent); Quality (rated Poor to Excellent); Service (rated Poor to Excellent); Value for money (rated Poor to Excellent)
  • Your comments: What went well?; What could we do better?; Any other comments
  • Follow-up (optional): Name; Email or phone; Agreement: “You may quote my comments (without my contact details).”; Agreement: “I would like someone to contact me about my feedback.”

Tips

  • Hand out paper feedback forms at the end of the visit, not afterwards; response rates drop sharply once people leave.
  • Rename the rated areas to suit you, such as “Food”, “Speed” and “Cleanliness” for a café.
  • Act on what you hear and tell people what changed; it makes the next round of feedback better.

Which feedback form should I use for customers or events?

For businesses, the customer feedback form adds questions about the purchase and staff. For events, the event feedback form asks about the venue, sessions and organisation.

Can I change the questions on the feedback form?

Yes. Rename any question, change its answer type (short answer, tick boxes, rating, table and more), move it, or delete it. You can also add whole new sections.

Can I put the feedback form on my website?

Yes. Choose “HTML” to copy ready-made form code with labels and a simple style. Set the form’s action to the address that should receive submissions, such as your own form handler.

Can I get the feedback form as a Word document?
