About this application form

Use a general application form when people need to be selected: a volunteer program, a community grant, a residency, a committee seat, a training scheme or a competitive membership. It gives every applicant the same questions, which makes applications fair to compare.

The template covers personal details, education and experience, why the applicant wants to take part, two referees and a declaration. For hiring, use the job application form; for schools and scholarships, use the student and scholarship forms, which ask the questions those panels expect.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. Frequently asked questions

How do I make applicants attach documents?

Add a “Tick boxes” question listing the documents you need (ID, certificates, portfolio) so applicants confirm what they’ve included. If they apply by email, ask them to attach the files alongside the PDF.

Can I score applications on the form?

Use the “For office use only” box at the end: add labels such as “Score”, “Reviewer” and “Decision” so reviewers can record results on the same page.

Can people fill in this application form on a phone?

Yes. Choose “Fillable PDF” and send the file. It opens in the Files or Drive app on Android, Files on iPhone, and any browser on a computer, where people can type their answers and save the result.

Can I add my logo to the application form?

Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.

Is anything I type sent to a server?

No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
