About this registration form

A registration form records who has joined something and how to reach them. It’s the first piece of paperwork for sports clubs, hobby classes, gyms, community programs, coaching centres and memberships of every kind.

This template collects contact details, an emergency contact, how the person heard about you and any needs you should know about, then ends with a signed declaration. Change the “Program” question to list your own classes, batches or membership types.

What’s on the registration form

The template has 5 sections and 17 questions. Every one can be renamed, moved or deleted.

  • Personal details: Full name; Date of birth; Gender (choose one: Female, Male, Other, Prefer not to say); Phone number; Email address; Address

Tips

  • List your options (classes, timings or membership plans) in the “Program” question so people tick instead of write.
  • If people pay on registration, add a short payment section with amount, method and receipt number.
  • For one-off events such as a workshop or conference, the event registration form is a better fit.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. What’s the difference between a registration form and an application form?

A registration form signs someone up for something open to them, such as a class or membership. An application form asks to be selected, so it collects qualifications and reasons that you review before saying yes.

Should a registration form include an emergency contact?

For anything physical or involving children (sports, classes, trips), yes. It’s included here and you can delete it for things like a book club.

Can I get the registration form as a Word document?

Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.

Can I change the questions on the registration form?

Yes. Rename any question, change its answer type (short answer, tick boxes, rating, table and more), move it, or delete it. You can also add whole new sections.
