About this survey form
A survey asks a group of people the same questions so you can count and compare the answers. Most questions are closed (a rating or a choice) because those are quick to answer and easy to tally; one or two open questions at the end catch what the numbers miss.
This template starts with a little about the respondent, moves to satisfaction ratings and multiple-choice questions, and ends with open comments. Swap the sample questions for your own topic and keep the answer types: they’re what makes the results easy to add up.
What’s on the survey form
The template has 3 sections and 10 questions. Every one can be renamed, moved or deleted.
- About you: Age group (choose one: Under 18, 18–24, 25–34, 35–44, 45–54, 55–64, 65+); How often do you use our service? (choose one: First time, Occasionally, Monthly, Weekly, Daily)
- Your experience: Overall, how satisfied are you? (rated Very dissatisfied to Very satisfied); How easy was it to get what you needed? (rated Very hard to Very easy); How likely are you to recommend us to a friend? (rated 0 to 10); Which of these matter most to you? (tick all that apply: Price, Quality, Speed, Friendly staff, Location, Range of choice)
- Comments: What did we do well?; What could we improve?; Name (optional); Email if you’d like a reply (optional)
Tips
- Use the same 1–5 scale for every rating so the results can be averaged and compared.
- Ask one thing per question: “Was the staff friendly and quick?” is really two questions.
- Keep it to about ten questions; long surveys get rushed answers.
Four ways to save it
A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. Frequently asked questions
What’s the difference between a survey and a questionnaire?
People use the words loosely. Here, the survey form uses mostly ratings and choices that are easy to count, while the questionnaire uses more open, written questions.
How do I add up the results from paper surveys?
Number each question, then tally the answers in a spreadsheet: one row per survey, one column per question. The create spreadsheet tool works well for this.
Can I add my logo to the survey form?
Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.
Can I change the questions on the survey form?
Yes. Rename any question, change its answer type (short answer, tick boxes, rating, table and more), move it, or delete it. You can also add whole new sections.
Can people fill in this survey form on a phone?
Yes. Choose “Fillable PDF” and send the file. It opens in the Files or Drive app on Android, Files on iPhone, and any browser on a computer, where people can type their answers and save the result.
