About this employee onboarding form
Good onboarding turns a new hire into a productive, confident team member quickly; poor onboarding is one of the main reasons people leave in their first months. An onboarding form makes sure nothing is forgotten: contracts signed, laptop ready, accounts working, people introduced and training booked.
This template is a checklist split into before day one, day one and the first month, with who is responsible and a date for each task, plus sign-off from the manager and the new employee.
What’s on the employee onboarding form
The template has 5 sections and 14 questions. Every one can be renamed, moved or deleted.
- New employee: Name; Job title; Department; Manager; Start date; Buddy or mentor
- Before day one: Paperwork (tick all that apply: Offer letter signed, Contract signed, ID and documents copied, Bank and tax details received, Employee information form completed); Set-up (tick all that apply: Laptop or computer ready, Email account created, System and software access, Desk, locker or workspace, ID card or access badge, Uniform or safety equipment)
- Day one: Day one (tick all that apply: Welcome and office tour, Team introductions, Health and safety briefing, Company policies explained, Role and expectations discussed, First week plan shared)
- First month: Training plan table (Training or task, Responsible, Due date, Done); Check-ins (tick all that apply: End of week 1, End of week 2, Day 30 review)
- Sign-off: Notes; Manager signature with date; Employee signature with date
Tips
- Prepare equipment and accounts before day one so the first morning isn’t spent waiting.
- Assign a buddy who isn’t the new hire’s manager.
- Schedule 30-day and 90-day check-ins on the calendar on day one.
Four ways to save it
A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. The Word file can be edited further, and the HTML code adds the same questions to a website. Browse more form templates, or design a completely different layout in the custom form builder.
Frequently asked questions
How long should onboarding take?
Paperwork and equipment take a day or two, but full onboarding usually runs 30 to 90 days, until the person works independently. The form covers the first month.
Who should keep the completed onboarding form?
HR usually files it with the employee record, and the manager keeps a copy until onboarding ends.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
Can I change the questions on the employee onboarding form?
Yes. Rename any question, change its answer type (short answer, tick boxes, rating, table and more), move it, or delete it. You can also add whole new sections.
Can people fill in this employee onboarding form on a phone?
Yes. Choose “Fillable PDF” and send the file. It opens in the Files or Drive app on Android, Files on iPhone, and any browser on a computer, where people can type their answers and save the result.
