About this interview evaluation form

An interview evaluation form (or scorecard) records how each interviewer rated a candidate against the same criteria, with notes and a recommendation. It makes hiring decisions more consistent, reduces bias and lets a panel compare candidates objectively after a long day of interviews.

Change the criteria to the skills the job really needs, keep the five-point scale, and fill the form in straight after each interview while it’s fresh.

What’s on the interview evaluation form

The template has 4 sections and 17 questions. Every one can be renamed, moved or deleted.

How many criteria should an interview scorecard have?

Five to eight. More than that and interviewers start guessing. Use the ones that predict success in the role.

Should candidates see their scores?

Usually not, but in some countries candidates can request their data, including interview notes. Keep notes professional and factual.

Can I add my logo to the interview evaluation form?

Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.

Can I get the interview evaluation form as a Word document?

Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.

Is anything I type sent to a server?
