About this employee feedback form
An employee feedback form asks your team how things really are: whether they have what they need to do their job, how they feel about their manager and workload, whether they see a future in the organisation, and what they’d change. Run it once or twice a year, or after big changes.
People are more honest when they’re anonymous, so the name is optional and the form asks only for a broad team rather than a role.

Four ways to save it
A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. Frequently asked questions
Should employee feedback be anonymous?
For satisfaction surveys, yes. Avoid questions (like exact role and start date) that together could identify someone in a small team.
How is this different from a performance review?
A review is the manager’s feedback to the employee. This form is the employee’s feedback to the organisation.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
Can I add my logo to the employee feedback form?
Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.
Can people fill in this employee feedback form on a phone?
