About this course registration form

Colleges, training institutes and adult-education centres use a course registration form each term to record which courses a student is taking, the credits, the timetable and the fee status, with an academic advisor’s approval. It’s also useful for language schools and hobby academies offering several classes.

Plan the term itself with the semester or academic planner once registration is confirmed.

What’s on the course registration form

The template has 5 sections and 15 questions. Every one can be renamed, moved or deleted.

  • Student: Full name; Student ID; Programme or major; Year (choose one: 1st, 2nd, 3rd, 4th, Other); Email; Phone
  • Term: Term (choose one: Semester 1, Semester 2, Summer, Other); Academic year; Registration type (choose one: New registration, Add / drop change)
  • Courses: Courses table (Course code, Course title, Credits, Day and time, Instructor); Total credits
  • Fees: Fee status (choose one: Paid, Partly paid, Scholarship, Pending); Receipt number
  • Signatures: Student signature with date; Advisor signature with date

Tips

  • Check prerequisites before submitting.
  • Keep total credits within your programme’s limits.
  • Keep a copy of the approved form until the term’s results are out.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. The Word file can be edited further, and the HTML code adds the same questions to a website. Browse more form templates, or design a completely different layout in the custom form builder.

Frequently asked questions

Can I add or drop a course later?

Most institutions allow changes during an add/drop period. Submit a new form marked as a change.

Who needs to sign?

The student and usually an academic advisor or head of department. Some courses also need the instructor’s permission.

Can I get the course registration form as a Word document?

Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.

Is anything I type sent to a server?

No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.

Can I put the course registration form on my website?

Yes. Choose “HTML” to copy ready-made form code with labels and a simple style. Set the form’s action to the address that should receive submissions, such as your own form handler.
