About this questionnaire
A questionnaire is a set of written questions used to gather facts and opinions in depth: for a school project, a research study, a needs assessment, a pre-interview screen or a customer discovery call. Compared with a quick survey, it gives more room for written answers.
The template includes a short consent statement for research use, background questions, a mix of yes/no, multiple-choice and open questions, and a closing question that invites anything the respondent thinks you missed.
What’s on the questionnaire
The template has 4 sections and 12 questions. Every one can be renamed, moved or deleted.

Tips
- Pilot the questionnaire with two or three people first; confusing questions show up immediately.
- Put easy factual questions first and sensitive questions later, once people are comfortable.
- Avoid leading questions such as “How much did you enjoy…?”; ask “How would you describe…?” instead.
Four ways to save it
A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. Frequently asked questions
How many questions should a questionnaire have?
Enough to answer your research question and no more. Most good questionnaires take 10 to 15 minutes; if yours runs longer, split it or cut questions you won’t analyse.
Do I need consent for a research questionnaire?
Usually yes, especially in schools, universities and health settings. The template includes a consent statement; your institution may require its own wording.
Can people fill in this questionnaire on a phone?
Yes. Choose “Fillable PDF” and send the file. It opens in the Files or Drive app on Android, Files on iPhone, and any browser on a computer, where people can type their answers and save the result.
Can I put the questionnaire on my website?
Yes. Choose “HTML” to copy ready-made form code with labels and a simple style. Set the form’s action to the address that should receive submissions, such as your own form handler.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
