A field trip permission slip tells families where the class is going, when, how they’ll travel, what it costs and what to bring, and asks for a signed permission to attend. It also collects the medical notes and emergency contact the teachers need on the day.

What’s on the field trip permission form

The template has 2 sections and 16 questions. Every one can be renamed, moved or deleted.

Tips

• Set a return date for slips at least a week before the trip.
• Take a copy of every slip, with medical notes, on the trip.
• Offer help with costs discreetly so no child misses out.
