About this student evaluation form
A student evaluation form summarises how a student is doing beyond marks: reading, writing and maths skills, effort, behaviour, working with others and independence. Teachers use it for term reports, parent-teacher meetings and when referring a student for extra support.
The strengths and goals sections are the heart of it; parents read those first.

The template has 5 sections and 18 questions. Every one can be renamed, moved or deleted.

Tips
- Write at least one specific strength for every student.
- Make goals concrete: “Read for 15 minutes daily” rather than “Read more”.
- Compare with the student’s own self-assessment before parent meetings.
Four ways to save it
