About this event waiver form
Fun runs, cycling events, treks, sports tournaments, adventure camps and hands-on workshops carry some risk. An event waiver asks each participant to confirm they’re fit to take part, understand the risks and agree to follow safety instructions, and records medical information and an emergency contact for the day.
Waiver law varies widely; this template is a starting point to adapt with local advice. For non-event activities, use the general waiver form.
What’s on the event waiver form
The template has 5 sections and 16 questions. Every one can be renamed, moved or deleted.
