About this assignment submission form

An assignment submission form, or cover sheet, goes on top of homework, projects and coursework. It identifies the student, the course and the task, records the submission date, includes an academic honesty declaration and leaves space for the teacher’s mark and feedback.

It’s used for both homework and larger assignments; teachers can pair it with a marking rubric from the rubric generator.

What’s on the assignment submission form

The template has 4 sections and 15 questions. Every one can be renamed, moved or deleted.

  • Assignment: Assignment title; Due date; Submitted on; Word count; Type (choose one: Homework, Essay, Project, Lab report, Presentation, Other)

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. A statement that the work is the student’s own, that sources are credited and that it hasn’t been copied or submitted before.

Can students submit it digitally?

Yes. They can fill in the PDF and attach it to the front of their file before submitting.

Can people fill in this assignment submission form on a phone?

Yes. Choose “Fillable PDF” and send the file. It opens in the Files or Drive app on Android, Files on iPhone, and any browser on a computer, where people can type their answers and save the result.
