About this student absence form
When a student misses school, most schools need a written note from a parent explaining the dates and reason. A student absence form makes it quick: tick the reason, enter the dates, sign. It also works in advance for planned absences such as family events, so the teacher can send work home.

- Absence: Type (choose one: Unplanned (already absent), Planned absence); From; To; Number of days; Reason (choose one: Illness, Medical appointment, Family emergency, Religious observance, Family event, Other); Details (optional); Medical certificate attached (yes or no)
- Parent or guardian: Name; Phone; Parent or guardian signature with date

Tips
- Attach a medical certificate for long illnesses if your school requires one.
- For planned absences, ask the teacher for work to catch up on.
- Keep the reason brief; schools don’t need private medical details.
