About this event permission form

Whenever children attend an event away from home without their parents, organisers need written permission: a dance competition, a youth camp, a sports tournament, a religious retreat or a sleepover. The event permission form sets out the event, supervision and transport, and collects the parent’s consent, medical notes and contacts.

What’s on the event permission form

The template has 3 sections and 14 questions. Every one can be renamed, moved or deleted.

• Event: Event; Dates; Location; Supervising adults and phone; Transport (choose one: Organised by event, Parents drop off and collect, Other)
• Child: Child’s name; Age; Allergies, medication or medical needs
• Permission: Agreement: “I give permission for my child to attend this event.”; Agreement: “In an emergency, I consent to first aid and medical treatment if I cannot be reached.”; Photos of my child may be taken and shared by the organisers. (yes or no); Parent or guardian name; Phone during the event; Parent or guardian signature with date

Tips

• Name the supervising adults and how to reach them.
• Ask about bedtime routines and medication for overnight events.
• Keep a copy with the group leader during the event.
