About this library book request form
A library book request lets members suggest a book for the library to buy, reserve a book that’s on loan, or ask for an inter-library loan. It records the title, author, ISBN and why the book is wanted, and lets the librarian note the decision.
Students can keep track of what they read with the reading log and book tracker printables.
What’s on the library book request form
The template has 3 sections and 10 questions. Every one can be renamed, moved or deleted.
- Requested by: Name; Class / department; Phone or email; Date
- Books: Books table (Title, Author, ISBN or edition, Copies); Request type (choose one: Buy for library, Reserve when returned, Inter-library loan); Reason or use
- Library use: Decision (choose one: Ordered, Reserved, Not available, Declined); Notes; Librarian signature with date
Tips
- Include the ISBN if you know it; it avoids getting the wrong edition.
- Teachers can request class sets for a unit.
- Librarians can use the requests to guide purchasing.
Four ways to save it
A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. The Word file can be edited further, and the HTML code adds the same questions to a website. Browse more form templates, or design a completely different layout in the custom form builder.
Frequently asked questions
Can I request several books on one form?
Yes. The book table has room for several titles.
How will I know if the book is available?
The librarian fills in the decision section and contacts you with the result.
Can I put the library book request form on my website?
Yes. Choose “HTML” to copy ready-made form code with labels and a simple style. Set the form’s action to the address that should receive submissions, such as your own form handler.
Is anything I type sent to a server?
No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.
Can I get the library book request form as a Word document?
Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.
