About this student registration form

A student registration form (also called an enrollment or admission form) is the first document a family completes when a child joins a school, tuition centre, coaching class or after-school program. It collects the student’s details, the previous school, both parents or guardians, emergency contacts, health information and which documents have been submitted.

Schools in India often also need the student’s Aadhaar or APAAR ID, category and mother tongue; add those as short answer fields if you need them.

What’s on the student registration form

Is a registration form the same as an enrollment form?

In practice, yes. Some schools “register” interest first and “enroll” after admission, but both forms collect the same details.

What documents are usually needed for admission?

Commonly a birth certificate, previous school report or transfer certificate, photos, proof of address, and ID for the student and parents. The checklist lists these.

Can I get the student registration form as a Word document?
