About this library registration form

School, college, community and office libraries register members so books can be issued and returned. The form collects the member’s details, membership type, an ID, reading interests and agreement to the library rules on loan periods, fines and damaged books.

Members can request books the library doesn’t have with the library book request form.

What’s on the library registration form

The template has 3 sections and 11 questions. Every one can be renamed, moved or deleted.

  • Member: Full name; Class / department; ID or roll number; Phone; Email; Address
  • Membership: Membership type (choose one: Student, Staff, Family, Community); Reading interests (tick all that apply: Fiction, Non-fiction, Comics, Science, History, Biographies, Magazines)
  • Library rules: Agreement: “I agree to follow the library rules.”; Member signature with date; Parent signature (for children) with date

Tips

  • Print your borrowing rules in the note on the form.
  • Record the library card number in the office box.
  • Ask parents to sign for young children.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. The Word file can be edited further, and the HTML code adds the same questions to a website. Browse more form templates, or design a completely different layout in the custom form builder.
