About this assessment form
An assessment form records whether someone meets a standard: a new hire’s skills after training, an apprentice’s competencies, a volunteer’s readiness, or a client’s needs at the start of a service. Each item is marked on a level scale, with notes on the evidence.
This template has an assessment table where you list the skills or criteria, a competency rating, observations, an overall outcome and an action plan with a review date.
What’s on the assessment form
The template has 4 sections and 12 questions. Every one can be renamed, moved or deleted.
- Assessment details: Name of person assessed; Assessor; Date; Role, course or unit; Assessment method (tick all that apply: Observation, Practical test, Written test, Interview, Portfolio)
- Skills and criteria: Criteria table (Skill or criterion, Level (1–4), Evidence and notes)
- Result: Overall outcome (choose one: Competent, Not yet competent, Further assessment needed); Strengths; Action plan; Review date
- Signatures: Assessor signature with date; Person assessed signature with date
Tips
- Write criteria as observable actions: “Sets up the machine safely” rather than “Understands safety”.
- Record evidence (observed, tested, discussed) for each item.
- Agree the action plan with the person assessed and set a review date.
Four ways to save it
A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. Frequently asked questions
Can I use this as a needs assessment?
Yes. Rename the skills table to “Needs”, change the rating to “No need / Some need / High need” and use the action plan for the support you’ll provide.
How many skills should one assessment cover?
Around 5 to 12. Change the number of table rows in the question settings, or split large assessments into several forms.
Can people fill in this assessment form on a phone?
Yes. Choose “Fillable PDF” and send the file. It opens in the Files or Drive app on Android, Files on iPhone, and any browser on a computer, where people can type their answers and save the result.
Can I get the assessment form as a Word document?
Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.
Can I change the questions on the assessment form?
Yes. Rename any question, change its answer type (short answer, tick boxes, rating, table and more), move it, or delete it. You can also add whole new sections.
