About this evaluation form

An evaluation form judges something against agreed criteria: a volunteer’s performance, a training session, a supplier, a program or a pilot project. Rating each criterion on the same scale keeps evaluations consistent between reviewers and over time.

Replace the five sample criteria with your own, keep the strengths and improvements boxes, and finish with an overall rating and signatures. More specific versions exist for employees, teachers, students, training and projects.

What’s on the evaluation form

The template has 4 sections and 15 questions. Every one can be renamed, moved or deleted.

  • Details: Name of person or item evaluated; Evaluated by; Period or date; Role, program or category

Tips

  • Define each criterion in a few words so different evaluators interpret it the same way.
  • Give examples in the comments, not just ratings: “Answered every call within a day” beats “Good”.
  • Share the criteria before the period being evaluated, not after.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. Frequently asked questions

What’s the difference between an evaluation and an assessment?

Assessment usually measures what someone knows or can do; evaluation judges the value or quality of something. In practice the words overlap, and both forms here can be adapted either way.

Which rating scale should I use?

Five points (Poor to Excellent) is the most common and easy to understand. Change the scale labels in the question settings if you prefer numbers or a different wording.

Is anything I type sent to a server?

No. The editor runs in your browser and builds the file on your device. Atifo has no server that could receive your form or the answers on it.

Can I get the evaluation form as a Word document?

Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.

Can I add my logo to the evaluation form?

Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.
